Initial Comments:
Based on the findings of an off-site follow-up survey conducted on March 23, 2023, New Life Home Care, Hospice & Palliative Care, has corrected the deficiency cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiency was cited as a result of a state re-licensure survey completed January 18, 2023.
